When you register and log in to Healm, you’ll find information about the National DPP lifestyle change program in a variety of formats, including interactive learning modules and videos.

Explore the full Program Pathway

As you work through the content, you’ll be asked a series of questions about your organization. Your answers will allow the platform to personalize your experience. They will also populate a Dashboard that you can use to measure your success after you adopt or expand your use of the National DPP lifestyle change program. You can update this information as needed over time.

Collaborate with experts and other employers along the way

We know it’s often more meaningful when you hear from someone in a role like yours who has been through the planning and implementation process. We’ve got you covered!

The National Association of Chronic Disease Directors (NACDD) and the Centers for Disease Control and Prevention (CDC) have created a network of employer champions called the Employer Network. Healm gives you access to this network, which allows you to participate in webcasts and discussions with these champions.

Through the Employer Network, employers can work together to prevent type 2 diabetes and improve workforce health across the United States. The network can help you make the National DPP lifestyle change program a covered benefit for your employees. It also allows you to share information with other employers and meet with subject matter experts from NACDD and CDC.

Healm Guides are available!

When you register for Healm, you’ll be connected to a trained Guide. You’ll receive support and expertise to help you make decisions about how to cover the National DPP lifestyle change program. Most of our Guides are diabetes staff in State Health Departments who work with local business coalitions on health or chambers of commerce. They’ve been working in your states and communities to reduce type 2 diabetes in the workforce and make connections with partners like you.

Guides will:

  • Monitor your progress and connect you to state and local resources, as needed.
  • Anticipate times you may need additional help and proactively reach out to you.
  • Answer support tickets submitted to Healm.
